UTILITY HIT / INCIDENT REPORT (MMS-003-UIR): Field data collection tool utilized by both subcontractors and prime to quickly investigate and determinate liability of damaged underground utility cut.  Field personnel will digitally document if the cable was properly marked (within 24” of underground utility) and document and/or notify liable party in accordance with Government SOW and contract.
